Public health services are available to residents and visitors. We do not deny services based on a person’s race, color, sex, disability, religion, sexual orientation, or national origin.
To receive certain services from Sumter County Health Department, you must be financially screened. Please bring the following items with you:
- Driver's License or other picture identification
- Any insurance cards
- Four (4) weeks most current/consecutive pay stubs for anyone working in the family household.
- No income: Unemployment letter or letter of support from the person who is paying or helping to pay your expenses for the month with a dollar amount listed in the letter.
- Financial Eligibility Determination is offered to clients to determine if a sliding fee scale based on household income is applicable. If you are eligible, charges will be adjusted.
